The Access Network meets multiple times a month with different network members hosting each meeting.

 

Having different hosts exposes network members to service agencies they may not have been exposed to before, broadening their understanding of what each network member brings to the table.

The demands of frontline workers are increasing as the needs of our most critical clients are becoming more complex.

INTERAGENCY CASE COORDINATION MODEL

One of the key goals of the Access Network is to coordinate efforts when working with some of the most vulnerable people accessing services from various service providers in the network. The Access Network utilizes a constellation collaboration approach, where clusters of sub-networks form as service teams.

 

Service teams would consist of network members connected to the vulnerable person, external partners also connected to the person, and other relevant supports (including cultural and/or wellness professionals). These support teams would then remain connected to the vulnerable person until their plan was complete, or until the plan was unsuccessful, requiring reassessment and the creation of a new plan.
